Case Scenario of a patient suffering from fatty liver disease,12th April 2015

29 years old Male, came to our liver clinic with complaints of pricking sensation in right upper abdomen and fatigue since 3 years. His Basal Metabolic Index (BMI) was 31 suggestive of obesity. On evaluation outside, he always had raised liver enzymes persistently. He had no addictions. His father was recently detected to be diabetic. We found that he had nonalcoholic steatohepatitis or in simple words, fatty liver damage. Fibroscan was 11.2 kPa suggestive of advanced fibrosis (Fibrosis stage 3/4). He also was developing insulin resistance, a feature of pre-diabetes. He also had increased cholesterol and triglycerides. He was started on strict diet plan and exercise to reduce weight. Medicines were started which can halt and reverse the damage. He is on six monthly follow up and is doing extremely well. The pain in the right side of abdomen has reduced, fatigue improved, insulin resistance reducing and liver enzymes have normalized.
